There are no pre-existing condition requirements with group health carriers. Rates are based on the company's industry type, the location of the business, and the average age of the employees (some carriers may adjust rates based on the health of a groups members). Benefit plan designs from HMOs, Point-of-Service, and Preferred Provider Organizations are available with different copayments, coinsurance, and deductible levels.
