Last day to submit Aetna group enrollment for an April 1, 2008 effective date

Last day to submit UPMC Medical Questionnaires and UC-2 to obtain medically underwritten rates for May 1, 2008

Last day to submit Highmark group enrollment for a May 1, 2008 effective date

Last day to submit Health America group enrollment for an April 1, 2008 effective date

March 25
Last day to enter, change or submit Health America One applications in BeneftExpress for an April 1, 2008 effective date

March 28
Last day to notify Health America One of applicant acceptance of rates for April 1, 2008 effective date BeneftExpress
**Please note, in cases where an offer is made to the applicant after the first of the following month, notification of applicant acceptance is due to ARMS Insurance Group within two (2) business days from agent’s receipt of offer.


**Highmark Individual Deadline**

Highmark will not guarantee effective dates, but with online enrollment, you can submit applications up to five days prior to the end of month and still REQUEST the first of the following month effective date.

ARMS recommends online submission before the 20th of the month to allow for processing time. Please note, applications with medical conditions will require more underwriting time; thus earlier submission is advised.
