June 28 Last day to submit Highmark group enrollment for an August 1, 2010 effective date June 30 Last day to submit UPMC Medical Questionnaires and UC-2 to obtain medically underwritten rates for August 1, 2010 **Highmark Individual Deadline** Highmark will not guarantee effective dates, but with online enrollment, you can submit applications up to five days prior to the end of month and still REQUEST the first of the following month effective date. ARMS recommends online submission before the 20th of the month to allow for processing time. Please note, applications with medical conditions will require more underwriting time; thus earlier submission is advised.
